High School Athletic "Hall of Fame" Nomination Form

Individuals should be nominated for extraordinary accomplishments for Upland Christian Academy athletics either as a player, coach, or contributor. A player must be a graduate of Upland Christian Academy who has been out of high school for a minimum of five years, a coach must be retired unless there are extenuating circumstances for his or her nomination, and a contributor is an individual who might have done things either behind the scenes or who has been involved in a variety of supportive roles.
